I just missed a mastered vital blow 10 times in a row fighting a perfect match mob. Granted these are rare, but 4~5 misses in a row isn't that uncommon. Is there any other factors taken into account for this skill? Or is it just supposed to be kind of difficult to land? If the second... why?

Like most spec skills these days, it relies partially on the underlying weapon ability, in this case Hand to Hand. This was part of the change where we removed the 90% minimum to specialize, though it's rare that a spec'd warrior doesn't accidentally master that skill very quickly.

It's a small effect, though. The rest is standard warrior-y stuff and the random number generator. It will not land 100% for any character under the current system. It's not broken.
