I don't think there was anything offensive about Cad's post. I'd say odds are the name thing is a death_angel glitch purely on the basis that it seems unlikely that someone with the exact same title, class, level and race would happen to show up here, unless of course the whole suggestion of auto deletion is a glitch on death_angel's part. To suggest that all of that proves nothing with the exception of it all being a glitch, is just silly.
I'm curious as well one way or the other, though I didn't interact with her at all in character.
