I recall a time (but my memory is untrustworthy since this is decades ago) where any death, regardless of source caused XP loss with infinite holes, but I don't recall when that stopped. I believe it went into 2nd and possibly stopped at 3rd age, but don't know for sure. Anyone here know?

I don't know exactly, but I'm 99% certain XP loss from PK deaths disappeared before the beginning of the third age and pretty confident it was not the case during most or all of the second.
It definitely predates the code being maintained within CVS.

The exp loss for PK stopped much sooner than the EXP cap was implemented. I recall back in late 90's building exp holes into the hundreds of thousands of exp tnl but it was only from mob deaths.
