There anyway to have two empty hands and wear something immediately to your offhand? Or do you have to wear two things and somehow get disarmed?

Duel wield is not "logical" it's old code and there are many poor things about it. What you ask can't be done, but perhaps one day we will get a right hand and a left hand. Who knows, we might be able to hold two items as well.
