Generally, fixed. Various factors make pincer more or less likely to hit, but if you connect with both axes, it's generally two rounds. There are some exceptions, such as Shield of Ice in play.

Possibly. Bloodthirst can allow the thirsting character a chance to reduce their lag somewhat. I throw a pile of italics in to hopefully head off 500 panicked posts by people who haven't noticed this in the past. (It's not a recent addition.)

Without looking at the code...
I want to say there's a chance (no guarantee) of like round and a half of lag there.

I invented a word based on you, but I can see that the word has lost its meaning.
Nepenthish -The act of taking cruel amusement from causing frustrations in others though the use of extreme vagueness.
