As an ongoing process, Umiron and I created a database of the many, many ways newbie (level 1-10) characters have gotten themselves killed in Thera over a 12-month span. They're spread out over a huge number of methods, but thanks to Umiron's fancy database work, we've begun isolating the most lethal things that aren't so easily blamed on bad decisions by the recently deceased.
As of this reboot, the causes of ~12% of those deaths have been addressed in some fashion. While the changes are not intended to be entirely foolproof, you're likely to have a couple extra chances to say "Huh. This isn't going well." or "Well, that doesn't look like a good idea."
This is an ongoing process, and while we're likely never to get to the hundreds of things that kill one newbie per year, we should be able to refine the biggest "traps". Please keep in mind that the things that kill more skilled players at lower levels are not necessarily the same as the things that kill less-skilled players.
As an example of something we're not going to adjust: If you attack a non-aggressive NPC who is well above your level because he happens to hold a desirable item, and he kicks your face up and down his Lumberyard stairs for your trouble, we're chalking that up to "a learning experience about the Consider command and greed".

Maybe make it so that casting armor of living bone can't kill you, and will just make you unconscious if you cast it with too few hp? Same with the rate items of gear that hurt when you pick them up?
Basically to stop people eating double deaths.

doing wear all and eating a second death from the item which does damage and poisons you, then my ghost being poisoned and dying instantly on the tick.

I'm not against this change, but when you get to your corpse it is pretty much automatic to do a get all corpse. I wouldn't immediately do a wear all though for reasons like this though.

Having never played a Druid before I figured why not.. In wishing to keep the wilderness familiarity up, I attained what I needed as far as armor and my first level in the academy, then recalled to the grove to level up from there. Thought I'd kill myself some small prey stalking around to build a level or two. Attacking what seemed to be some easy camoed creatures, my fellow Druids there did not appreciate that at all and assisted the creatures, killing me outright in one or two quick rounds. One could argue that would cause some frustrations. I do know that Druids are defenders of the wilds and stuff, however they need to eat/circle of life philosophy too. Just one more way for a newer one to die when they think they are killing something harmless. Maybe have these druids give some warning? Until one can make it up enough to kill the influx of farmers and citizens to the west. At the least something to consider, thanks!

Considered as a few lucky and then proceeded to do some illuminate supp that did caps damage, 2 and I died. I won't make that mistake again.

So, just the other day I was walking down eastern and I saw my best buddy O' pal Prometheus sitting there invis. And he was still tracking someone. I have fallen victim to this guy with my noobs on eastern a few times. I have no idea why he tracks as far as he does (This is Prometheus from the troll castle in the past).
Perhaps you can add him to your list of tracking NPC's to look at.

I wonder if there's an easy way to set up something similar to an 'Apdex ' for CF - or some kind of metric to measure 'frustration'.
Essentially you'd need a good way to audit out the types of events that identify user behavior (dying, regearing, leveling, practicing, farming, empowerment praying, etc) - then assign an pseudo-arbitrary weight in terms of frustration to each one. Then you could narrow down common QoL concerns in an entirely objective fashion and tweak to match the profile you're looking for.
It wouldn't be perfect of course but I think it'd be a really interesting sort of thing to set up.
I also think it'd be cool if there was some kind of 'exit polling' we could incorporate - but that'd probably require you to have some kind of account that is shared between characters (which...might be fine...it's not like you can't figure out who is playing whom).

People being tracked by hostile mobs, which move to the edge of an area because they can't go any further. Then they quit out. As soon as the lowbie steps into the area, they get destroyed.
I've had dire wolves kill me at the gates of Udgaard (ironically when I was a trapper with gentlewalk, so I probably shouldn't have let myself die there). Sahuagin kill me when I walk across the open sea. Drow patrollers when you step into the underdark.
You don't really expect to need to scan every time you enter an area that 99.9% of the time is safe to walk into.
I think it would be great if mobs remembered who they were after through a quit, for maybe an hour rl time, and then returned to their usual spot.

You might remember dying to these things because you're a top-tier player and you're not dying very often. (I'm not saying there's no need for eventual action there, but there are other instances that are glaring.)
The #1 killer over the last 12 months was the rabid beast in the Academy, with a comfortable lead over his nearest competition (Tarus). We did a small tweak there, though we might need to go further. (e.g., The root problem may be newbies entering the Academy cavern before practicing in their guild.) The elf mage in Aldevari was a top-ten killer, largely because of Cyclone, so we made a tweak there that still preserves the "I'm fighting a mage!" experience. Etc. The prolific killers tend to be NPCs that will rarely kill a player like you.
(Except Tarus. He's just a hard-working businessman, minding his own affairs, constantly being forced to put on his kicking boots and make someone rage delete amid Immortal laughter.)

Yanoreth implemented something like this roughly a decade ago, and scanning the logs it happens fairly regularly. We might tweak it, but you're just having bad luck in finding them in the interim.

If a newbie is playing a paladin, and hasn't got the adrenaline timer going, maybe give them a warning about walking into Udgaard by the gates, the first time they do it, rather than let them get slaughtered by the guards.

It has killed my characters a couple of times too. It is particularly dangerous to elves and mages that have low hp/less defenses. I have recalled away from that beast several times, just to be on the safe side.
Some thoughts of what makes the rabid beast such an effective killer: -The rabid beast has a good hit roll and often hits its target. Remember that AC is the primary defense mechanism for the Academy level characters. If you go there before gathering the fine leather set...
-The rabid beast is placed in the middle of the path rather than end of the cave. The newbie gets disoriented as they flee to random direction and runs back to the rabid beast accidentally while having low hp and it finishes the job.
Tarus is likely due to greed of players who try to get charred bracers too early in the game.
