under her PK death section, entries like the following are present. This is the first time I've seen this? and, so I am curious of the cause?

etc..

Is that her servitor going ape-#### and killing her? Or, is it just some buggy records?

Two of my three deaths were the same. And they were due to me killing myself by casting armor of the bones *brainfarts*

So it is likely she killed herself in some fashion.

I imagine a familiar death by taking damage when they die must look like a self-kill ala armor of bones.
