Now that wouldn't be very useful now would it. Suppose your helping tribunal defend the Spire. The tribunals with you cannot heal you or hurt the person attacking. So they call you. You attack the person and they run, but you are wounded. Now before they come back full health, your going to need to heal up, but how can you because you've just all your heals from the pervious battle you were in. OR, suppose your attacking tribunal and need to heal up before you return to kill the person they have helping them? Either way, like you yourself said, if the healer doesn't care where his money is coming from, why would they limit the number of heals they would give out?

Higher level items of this type definitely exist, and my area knowledge doesn't include pretty much anything added in the last year.

Related to the first comment... imagine one hero Maran defending the Fortress from a group of Imperials. Imagine what a difference it'd make if three lowbie Maran were at the inner guard with him, spamming heal heal for him. This is more help than they reasonably should be able to give, unless they're healers or such themselves.
