I know there's a penalty for role playing an alignment change, but is there a drawback to getting your ethos changed... or is that even possible?

It is possible. If you do it *on purpose* it will involve you doing some quest for an Imm. If you get yours changed by an imm as punishment for poor role-play then yes, it will probably come with drawbacks.
