I think I remember Nepenthe posting something about Sea of Blood and how it broke trying to implement items sinking. This idea is similar: If you are in the air and your fly runs out, you fall. For example, in the air above Galadon, instead of being stuck, you fall when your fly runs out and you take massive damage or die in the streets of galadon. Makes perfect roleplay sense, and indeed it doesn't make sense with the current way it is. I can understand if it is too hard to implement.

I assume you are talking about shifter forms, and their air area. This already exists to some degree. There is no "fly running out" for air forms. There is however running out of mana and/or movement which will make them go crashing to the ground somewhere below.

Doesn't seem to me to have anything at all to do with roleplay since it's game mechanics. As I said...it already exists. So long, and thanks for all the fish!

It makes sense for a person to drop 'to the ground' when their flight falls (no matter what the reason for it falling is). Having your flying spell fall while in the air, only to remain in the air is nonsensical. I can imagine this being complex to implement, but it will add to the realism. Can you visualise a person stuck 30 feet in the air because their fly spell dropped? It could easily be something like what happens in Zulg's Barovia keep if you do not have the fly affect (i.e. you fall into Market Square, take moderate damage, and have a boneshatter-like affect).
I can't think of many other areas that have an "air room" like that above Galadon (Gareth's Keep has one room, I think), so I guess it'd be up to whoever takes care of Galadon updates whether to tweak the area. Given the rarity of these "air areas", though, probably wouldn't have much of an impact. Personally, it seems like a somewhat cool idea to add flavor to the "air above Galadon" if the area creator wanted to do it.
