I was wondering is it like the transmuter quest forms. . .asking for it is the surest way of not attaining it? Obviously I have never had one and was curious how one goes about doing that.

This was recently discussed over on Ask An Imm. Basically, establish a name for yourself as hero characters, and apply. There's various FAQs on the website, and if you scroll down a little on Ask An Imm you'll see a longer discussion.
