When I was playing a Arvam Pally and had a <tainted> Midnight Dragon eq on for like 10minutes, I was turned neutral.
This Paladin had tainted gear on for the longest time and I saw nothing from the IMMs. He started to mock me. So, I sent a note and prayer, but got no response.
I am sorry IMMs for pointing fingers at you and your busy schedule. But he was mocking every paladin who told him about the gear. I remember Gwergen (Fort Pally, sp?) tell me the same thing. Sorry, it was very frustrating.

Since he could have just cleansed it himself.
But yeah, my pallies have had echoes sent their way when they've inadvertantly worn evil-flag gear. I also make a point of cleansing people's gear whenever there's downtime. If they refuse, I stop travelling with them.
