1) It's not obvious from the log what caused the death. I archived the log, but there's no indication inside it about what caused it.

2) My best guess is: You have a huge pile of triggers running, parsing and altering the text all over the place. Given that we're not receiving any reports showing similar incidents with other people, I'm inclined to think that your trigger system was mangling the output you'd otherwise see. If you turn off that ridiculous system and still see a problem, report it again. Otherwise, I'll wait for a similar complaint from someone who is sending unedited text.
