I looked at him, and he was wearing that captain's hat. I killed him. No hat. I know some things can be rot death, but my experience has been that the item is there on death and crumbles a few ticks later. Here, there was just nothing at all, like he never had it.

This can happen when an item is overlimit, just like it would vanish from a players corpse.
