Was fighting the inner guardian of Outlander and was using avatar of venoms, saw some toxins get transferred to the inner.
I had to flee, but when I came back the inner was no longer in it's room. It was at the Circle of Thorns intersection, inner was killed but the Fetish didn't drop.
I'm guessing it didn't drop because the inner wasn't in it's room. I didn't think inners or outers were able to flee or move from their spots in the cabal.
Not sure what toxins/poisons were transferred but at least one of them was virulent venom and one that makes you crazy.
So the bug, I would have to say, has to be something involving the toxins from avatar of venom, getting transferred to the inner of Outlander, causing the inner to flee, however the inner didn't flee again after that.
