Not a fake copy/paste that makes it looks like 6 deaths, and then I'll look into it. Since you didn't die 6 times from one 'wear all' command (though the logs did show you were stupid enough to do it twice in a short period of time).
Trying a simple repro it works fine for me (only one death as woudl be expected), so I'm not sure why you're dying twice. So long, and thanks for all the fish!

The first time it happened it got cut out of the buffer, pasted this out of the "WTF, is Cador up there rage slaying me"
to recap, from death, picked up the sword, wielded it on accident the first time, killed me twice.
Some lowbie looted me hard and made me get gear for them, while ghost ran out in a "#### THIS DAY SUCKS" mood got the gear, died in the process, went back, wielded a polearm, but a goddamned demon pendant was in the inventory, made me drop it and for some damn reason WEAR ALL qued up that stupid sword, so guess what.....

You've gotta be dead, wear all with a jagged black saber in the inventory, and not sure if it matters there's another jagged short sword in the inventory too. WHO KNOWS, i don't know why it happened. Recall spot was hamsah.
