This example is most apparent in Darsylon, where the guards are sword specs and both weapons are equal weight. When you disarm a guard, he will 'get weapon;wield weapon' but for some reason he will not wield it, and the dual wield will drop to main wield, such as when the weapon doesn't complement your dual wield. So when you disarm an elite/immaculate guard, they will only wield one longsword, and have the second in their inventory.

If that doesn't make any sense, just disarm an elite swordsman and you'll see he doesn't re-wield.

Unfortunately this is by design...when mobs are loaded they can pretty much break any rule. When they are disarmed, and try to wield, they follow the same rules a PC would. Problem is mobs don't truly have specs, so wielding two of the same swords isn't possible as a mob.
