This happened to me for the Worm in the Ruined Keep, so I'm assuming the same thing would go for any shapeshifter snake forms, fish, etc etc.

Gouging targets arms, legs, and chest on them. Arguably, those types of creatures don't possess any of those body parts, so perhaps simply a change to gouge their body?
