I have it at 100% but failed five times in a row.

Is there any advantage to having 100% berserk over 75%? Does the 100% berserk offer more boost to hit/dam?

Apparently the random number generator hates you.

It fails less at 100% than 75%, but there are also additional factors that can cause it to fail/succeed.
