If a poisoner kills someone by suggesting that he use an item that result in his death, i.e. the poisoner himself doesn't directly cause the death, should that count towards the poisoner's PK ratio and cause him to distend? Does it already work that way?

1) It's difficult to track that, mechanically. If the poisoner Suggests you to drink an emetic, and the poison kills you six ticks later, the poisoner should get credit, but the code isn't very amenable to that, and they don't. It's possible to make a workaround for instant kills, but even that runs into problems.
2) At present, no the poisoner doesn't receive PK credit for it, and thus it doesn't factor into distention.
3) All that said, one of the more popular Suggest-ed items was relocated recently at the advice of another Immortal, and the difficulty of acquiring one is now more in line with the power of the item, rather than it being available to low-level characters with no real effort. That whole area has a lot of risk/reward issues, and it'll probably get a more thorough re-evaluation at some point.

Okay, as long as it's something that's on your (collective) radar. It seemed wack that a poisoner thief could kill tons of people and more or less ignore distention entirely.
