No, we don't automatically know. A certain level of immortal (56 or 58, I can't remember) can see IP addresses. Some people make it very clear they are the same character over and over and you don't even need IP addresses to confirm (constantly rolling air/offense majors and just trying to kill anything that moves). Personally, I avoid using IP addresses unless I'm dealing with punishment/administrator stuff. It just doesn't matter that much to me. There are others I'm sure who pay closer attention to that, but they have to keep track of characters and IPs on their own, there is nothing in the code to help them out.
