Shouldn't he have removed his weapon on the return blow?
In grapple, he took the weapon, he shouldn't be able to cleave with it if he doesn't have it at that moment. The return strike should be empty handed...

By design...


This skill is similar to Disarm and Strip, but has much less finesse in
that the Orc attempts to grapple a weapon from his opponent by any means
possible, often causing himself some pain in the process.Various levels
of success are possible when using this skill.


Grabbing a weapon out of someones hand is always going to cause you damage when you do it.
