This happened to Kharghurln (IIRC) between Acid and Blessing - basically, I had enough charges for the control, but I didn't get it (ended up praying and an Imm set it on me). Ravon just prayed about his not getting lightning after cold...he finally worked his way up to enough charges to get acid, but never got lightning (I set it on him). So basically, whatever code is checked upon a kill to see if you get the next control, must not be working 100% correctly. Maybe there's something associated with the kill that puts you past the limit that has a hole in it or something. Dunno.
