Just a thought.. any chance you're wearing some gear that gives you -CON? Or maybe you have some affect that is?
Also, certain illnesses which do NOT show up on affects list still reduce con somewhat. That might be the problem (If you keep seeing an echo that you cough or something, that's a clue).

Have you checked your gear for -con pieces? Tried being naked and shouting?
If you're not a fire giant, you don't have 23 con.
