Same way we handle NPCs that are too easy/hard for ranking. Some of it will come down to author intent. If they typo'd the damroll, it will get fixed. If the NPC is designed to be weak for a good reason, no. If the author isn't presently on staff, a higher-up will make the call.

If the NPC is weak for its level on purpose, we do have mechanisms to address that without necessarily making it the same as other NPCs of the same level. For example, some are worth more or less experience points, as you've probably seen in ranking.
