I don't really remember the details, but it was pre-me-on-staff, so, a pretty old copy of the code. Infrastructure in place for cabals and PK ranges, some of the old anti-paladin/ninja/ranger/paladin stuff, etc. I think depending on who you ask Morgath either gave the code away or was stupid enough to leave it somewhere people could just take it, and they did. BoltThrower would be the guy to ask for the story here.

Whole areas... probably not. I'd guess a decent significant chunk of Hell has never been seen, but I'm not really sure. There are certainly parts of other areas that are rarely seen whether or not they have been at some point.

Positive; we don't have one of those. I can see why you'd want one just for testing purposes; for whatever reason, Galadriel back in the day settled on just making very high level imm cancel/dispel nuke all affects regardless of what they are, instead. It works well enough for anything I'd want to do with an affstrip or whatever so there's never really been any incentive for me to put time into changing it.
