Here's a list of the changes to cranial and when. Not all of them were done by me, but hopefully this answers your question. I don't remember the beatdown you mentioned, but I know for a while there if you had two mace specs cranialing you were pretty much permalagged.
6/12/03 - victim's helmet was checking pierce AC for some reason, was changed to bash AC 9/18/03 - victim's helmet (if it met the AC requirement) helps out a little more (Note: Yanacek's helmets he wore never were high enough in bash AC to make a difference.. most of his life he had the crown of eldritch power, which is 0 armor) 11/6/03 - AC requirement for victim's helmet increased slightly 10/6/04 - cranial in combat has a timer like cranial out of combat does
